For Referring Providers
Ponte Vedra Behavioral Health provides applied behavior analysis (ABA) services for children and families. We collaborate with referring professionals to support coordinated, ethical care.
How to Refer
To refer a client, please contact our office with the information below. A member of our team will follow up to discuss next steps and availability.
- Client name and date of birth
- Parent or guardian contact information
- Reason for referral and primary concerns
- Insurance information, if applicable
- Relevant evaluation or treatment history (when available)
After We Receive a Referral
We contact the family to schedule an initial conversation, review clinical fit, and outline assessment and service options. With consent, we can share updates with referring providers as appropriate.
